There ought to be a rule:
No headphones or ear buds in BOTH ears.
One ear must remain uncovered/unplugged at all times.

Why do you think this is necessary?  What limit/game do you regularly play.  If someone continually slows up action, then yes.  But otherwise, I see no need for this.  I regularly listen to music while playing, and only time I clarify action is when someone doesn't have chips in front of them and are acting on a hand (when chips are en route).

A lot of the players w/ the headphones/ear buds slow things up.  It's a minor annoyance usually but there are a few players who are 'chronic'.  Anyway, I've got my new Bose noise reduction headphones that I absolutely MUST use when it's really noisy so I ain't gonna stop no how.  Besides, they don't interfere w/ me hearing voices.  Too bad, that.

I have more of a problem when people eat at the table.  I always eat when I play but I always pay attention too.

I think that most people can play fine with headphones on.  I can see where it may be worse at 5-150, but I don't play that game so I don't really know.

There are quite a number of players who DO pay enough attention and it hasnt been a problem. BUT, It seems to be becoming more of an issue as of late. 
Probably a bit of frustration vented due to a game where half the table didnt know what was going on 3/4 of the time because they all had headphones or earbuds in.  The dealer was going insane repeating things three or four times a betting round regarding the same action. Made for a really slow boring 5-150 game.

I haven’t seen game play being slowed by headphone users. But I certainly have seen it slowed by cell phone users.

Cell phone users not only are annoying because they chatter during the action of hands. But they also never seem to be able to act when the action gets to them.

I would set a house rule that the dealers cant deal new hands to players using cell phones.

Problem solved.

I think this is a great idea.  They did this during the ASPC and I thought it was a great rule.  I never saw any problems with phones the entire 13 hours that I played.

General House Rules
1. NO SMOKING. Smoking is permitted outside.
2. Drinking alcoholic beverages is permitted, as long as it is within reason and does not get out of control. No one under 21 is permitted to drink. Anyone who, in our opinion, has had too much to drink, WILL NOT DRIVE!
3. No eating is permitted at the poker tables. If you eat elsewhere, please wash your hands before returning to the table.
4. Any drinks at the poker tables will be either in your hand or away from the table. Exercise care when drinking at the poker table. You will be held liable and charged a 100 dollar felt replacement fee if you spill your drink on the table. This is regardless of actual damage done.
5. Treat other players with respect. Disorderly conduct will not be tolerated and you will be asked to leave if you cannot behave yourself.
6. Common poker etiquette applies at all times. Please become familiar with or we will be more than happy to educate you, harshly.
7. No weapons of any kind are permitted in the poker room. Anyone found with any type of weapon will be promply removed.
8. No one under 18 is allowed to play.
9. Music headphones will be allowed, but if they are a distraction, we will ask that you remove them.
10.  Cell phones are to be silenced.  We ask that if you need to make use your cell phone, you do it in a way that is not disruptive.
